The columns are wrong due to selecting the wrong field names in the query.
Cause:
The field names in the 'Field' row of a query in Design view are not the desired ones or the field names are in the wrong order.
Solution:
In Query Design view, select the desired fields in the 'Fields' row or rearrange the columns in the query grid.
1) If the Database window is not active, activate the Database window.
2) Click the 'Queries' tab in the Database window.

3) Select the desired query in the list box. (The selected query is highlighted.)
4) Click 'Design'. (The query appears in Design view.)
5) Click in the desired column in the 'Fields' row of the query grid. (A down arrow appears.)
6) Click down arrow. (A drop-down list appears.)
7) Select the desired field name from the 'Fields' drop-down list.
8) Repeat steps 5) through 7) for each additional field in the query grid.
9) To change the order of the fields in the query grid, do the following:
a) Position the mouse pointer above the field name in the 'Field' row to be moved. (The cursor changes to a black down arrow.)
b) Click mouse button. (The selected column is highlighted.)
c) Hold and drag the mouse pointer to the desired location. (A vertical line moves with the mouse.)
d) Release the mouse button at the desired location. (The selected column appears in the new location.)
e) Repeat steps 9)a) through 9)d) for each column to be moved.
10) Select the 'File' menu and select 'Save' to save changes to the query.
